Patient Forms

Preparing for your procedure includes following the preoperative instructions Dr. Quatela and Dr. Lee provide, completing the necessary forms and, if needed, arranging for financing.

First, please complete the Patient Demographic Sheet below and bring it with you to your initial consultation appointment, along with a photo ID and insurance card. Although hair restoration procedures are not covered by insurance, our office is required to have a copy on file in case of an emergency.

If you prefer, you can complete the new patient paperwork electronically prior to your appointment using the secure patient portal. This assists our office, as we have transitioned to using electronic medical records.

Many of the individuals who choose the Quatela Center for Hair Restoration travel from outside the area to undergo hair transplant surgery. Our staff is committed to making your visit as convenient and comfortable as possible, and can help arrange transportation once you arrive in the Rochester area.
